Participation Forms

Please click link below for an updated physical form. Each student is required by state law to have a current physical on file in the activities office prior to participation in athletics, including cheer and dance.  Physicals are good for 13 months after the examination.  Please make sure that a parent signs the release on the back side of the physical.
